1. If a camper or camp staff member tests positive for COVID-19, parents/guardians will be informed.  That person will not be permitted to return to camp for two weeks in accordance with CDC guidelines.
    
2. If the Director tests positive for COVID-19, the facility will close for two weeks in accordance with CDC guidelines.
    
3. Should the center need to close for any reason, tuition will not be refunded or reduced for closures of less than 15 camp days.
    
4. No refunds in the case of camper withdrawal.

COVID-19 Risk of Transmission and/or Infection Acknowledgment, Assumption of Risk, Release, Waiver and Indemnification Agreement

This Acknowledgment, Assumption of Risk Release, Waiver and Indemnification Agreement is hereby executed by the adult(s) cited above, parent/parents of the child/children cited above.

My electronic signature below indicates that I have read and understand this agreement in its entirety. Having requested Temple Sinai, Inc. [Temple Sinai] to accept Child into one or more Temple Sinai summer camp, childcare, religious school, or early learning service program(s), Parent, having read and understood the below, hereby agrees, on behalf of Parent, Child and Child’s family, as follows.

I hereby understand and acknowledges that:

1. COVID-19 is a highly contagious and community transmitted disease currently considered a pandemic within the United States according to the World Health Organization and Centers for Disease Control and Prevention, active in the State of Florida and, specifically, Sarasota County.
2. Although Temple Sinai will utilize all reasonable efforts to maintain compliance with applicable COVID-19 prevention and sanitation guidelines, Temple Sinai cannot guarantee that persons including but not limited to Child and Parent will not become exposed to or infected with the COVID-19 virus.
3. Persons infected with COVID-19 may be asymptomatic preventing Temple Sinai from reasonably becoming aware of an associated infection and taking remedial steps regarding such asymptomatic person.
4. No assurances or guarantees have been made to Parent or Child regarding the potential transmission of COVID-19 while Parent or Child are on Temple Sinai premises or utilizing Temple Sinai program services.
5. No specific action or inaction taken by Temple Sinai could reasonably prevent the transmission of COVID-19 to Parent or Child while on Temple Sinai premises or utilizing Temple Sinai program services.
6. In spite of Temple Sinai’s best efforts to comply with applicable COVID-9 prevention and sanitation guidelines there exists a risk that Parent or Child may be exposed to or at risk of infection with COVID -19 while on Temple Sinai premises or utilizing Temple Sinai program services.

In consideration of Temple Sinai permitting my child/children to participate in Temple Sinai, Inc. program services I hereby agree, on behalf of Parent, Child and Child’s family, as follows:

1. We voluntary agree to assume all risks of exposure to or infection with COVID-19 and associated injury, illness, death or damages of any form and fashion related to physical presence on Temple Sinai premises or utilizing Temple Sinai program services.
2. We hereby release, discharge, hold harmless from liability and agree to indemnify Temple Sinai, Inc. a not-for-profit corporation and its representatives, employees, agents, officers and directors and including Camp Gan, The Gan at Temple Sinai, The Religious School of Temple Sinai, Camp Sinai from any and all injuries, illness, disability, death, or damages of any form and sort resulting from exposure to or infection with COVID-19 associated to Parent and/or Child’s presence on Temple Sinai premises or utilization of Temple Sinai program services.
