SAFETY Event: Mini Golf & Fun Afternoon!

Grades: 6–12

Cost: $20

Join SAFETY for an afternoon of fun, friends, and mini golf! We’ll enjoy a friendly round of mini golf, lunch, and plenty of time to hang out together.

Lunch is provided — please let us know if your child has any food allergies or dietary restrictions.

Transportation: Does your child need a ride? Let us know if they will need to carpool with another adult so we can help coordinate safely.

Parent Consent: A signed parent consent form will be required to participate. Please fill out the form below.

I give permission for my child to attend the October Safety Event at Evie's. I understand that my child will be under the supervision of Temple Sinai staff during the program. I also acknowledge that it is my responsibility to provide transportation to and from the event
